Minors attending their first FREE CLASS are required to have a RELEASE FORM signed by a parent. If you are a minor who will be attending without a parent present, please print and fill out the following form.  It may also be necessary for our staff to contact a parent to verify parental permission. For this reason, if possible please have a phone number where a parent can be reached. 

NOTE: It is a crime for a person to sign this form falsely representing that they are the PARENT/GUARDIAN for the student or for the member to falsely represent that they are over the age of majority (18 years).
